ENTRY OF DISMISSAL {¶ 1} On April 7, 2005, plaintiff was issued a return of complaint form/direction to complete form requiring her to submit a corrected complaint form providing the name and address of a proper defendant. Plaintiff has failed to comply with the court order. Therefore, plaintiff’s action is DISMISSED, without prejudice, pursuant to Civ.R. 41(B)(1). The court shall absorb the costs of this case. The clerk shall serve upon all parties notice of this entry of dismissal and its date of entry upon the journal.
