THE COURT FINDS THAT:

“1) On August 15, 2002, plaintiff, Richard M. Cochran, filed a complaint but failed to provide a complete mailing address for defendant;

“2) On September 5, 2002, plaintiff filed a corrected complaint but failed to name a proper defendant;

“3) On September 9, 2002, this court issued a pre-screening entry (Jr. Vol 718, Pg. 13) dismissing Public Utilities Department, Electricity Division as a party and requiring plaintiff to file a complaint form naming a state entity as defendant. The entry, in pertinent part, stated: “This complaint will be dismissed unless an amended complaint is filed on or before October 7, 2002.”;

“4) A check of the docket reveals plaintiff has not submitted the completed form complaint.

IT IS ORDERED THAT:

“1) Plaintiff’s case is DISMISSED without prejudice;

“2) The court shall absorb the court costs of this case.
