ON CERTIFIED REPORT by the Board of Commissioners on Grievances and Discipline of the Supreme Court, No. 91-46.
By an amended complaint filed December 9, 1991, the Office of Disciplinary Counsel charged respondent, Donald Wayne Conley (Attorney Registration No. 0033831), in four counts, with misconduct. The complaint alleged that respondent’s conduct violated former Gov.Bar R. VI(1) and (3) (now Gov.Bar VI[1], which requires every attorney to file biennially with the Clerk of the Supreme Court of Ohio a certificate of registration, with a registration fee), Gov.Bar R. V(5)(a) (which requires attorneys to cooperate and assist in disciplinary investigations), and Gov.Bar R. VII(2)(A) (which prohibits the practice of law by anyone not registered under Gov.Bar R. VI). Respondent’s answer filed January 14, 1992 admitted the factual allegations, denied the legal conclusions and averred that all registration fees had by then been paid.
At a hearing before a panel of the Board of Commissioners on Grievances and Discipline of the Supreme Court, an agreed stipulation of fact and recommendation for sanction that had been entered into by the parties was accepted by the panel, and testimony was presented. The record reflects that

respondent failed to comply with the requirements of registration and payment of fees, and to accept certified mail notices relative to such failure.
The panel found that respondent had failed to comply with the registration procedures for attorneys, that his illness had been a significant contributing factor to his repeated failure to comply, that he was cooperative when he finally responded to notices from Disciplinary Counsel, and that he acknowledged that his failure to respond to residential certified mail was inappropriate and compounded his problems. The panel recommended a public reprimand.
The board adopted the findings of fact, conclusions of law, and recommendation of the panel.
